As the only licensed “direct writer” in the Illinois physician professional liability market, our application process starts with a personal meeting because we want to know you and your practice and we want you to know us.
In order to determine your eligibility for coverage we also need the following underwriting information:
- Recent (complete) Illinois State Credentialing Form
- Summary report of procedures/patient encounters by CPT Code with RVUs if applicable
- Confirmation of your current malpractice insurance coverage
- Confirmation of claims history with your current or past carriers
